Yale Divinity Library Restricted Circulation Books, Pamphlets, or Microforms Request Form

Many of the Yale Divinity Library's restricted circulation holdings are shelved at the off-campus Library Shelving Facility (LSF). Please complete and submit this form at least two business days before you wish to view the materials.

Please note that if you do not have a Yale ID, you must respond to the confirmation link that you will receive via email after submitting the web form linked to above. Please submit the web form and respond to the confirmation email before you come to the library if possible.

In order to be cleared for examining restricted circulation material you must bring identification with you to the Special Collections Reading Room. Acceptable forms of identification are 1) a Yale ID or 2) a current passport or 3) two types of ID of which at least one must be a government-issued ID bearing your name, photograph, and an expiration date indicating current validity (e.g., driver's license, state-issued ID card). Second IDs for verification can include college IDs, credit cards, etc.

Patrons without borrowing privileges who wish to view off-site circulating items in the Special Collections Reading Room must also submit the form below and go through the registration process.
