Library staff may use this form to report problems associated with name, subject, series, and uniform title headings in Alma, including
- split files in heading index displays
- headings that have been assigned incorrectly
- headings that need additional cross references
- duplicate or conflicting authority records
- report problems with linked fields in records that contain non-roman script data (linked fields begin with $6)
Staff may also use this form to request the addition of a death date to an "open-ended" heading if
- they have received a patron complaint
- the person represented by the heading has strong associations with Yale or New Haven
- the person represented by the heading will be the subject of a local event (conference, lecture, etc.)
